Fertility isn’t exactly dinner-table conversation. Most guys never think twice about their sperm count, not until months of trying for a baby turn into something more frustrating than they expected. That’s usually when someone finally says the words “semen analysis test” out loud. The funny thing is, the body’s often been hinting at this way earlier. You just have to actually be paying attention.
Sign 1: A year in, and nothing’s happening
This is the point where most guys stop shrugging off a semen analysis test. Doctors generally say twelve months of trying, or six if your partner’s past 35. It’s not some magic number. It’s just a reasonable line where “let’s give it more time” stops making sense and “let’s find out what’s going on” starts to.
Sign 2: Something about your semen seems different
Could it be less thick? Could it just be smaller than you remember it, or could the colour have shifted in a way you can’t quite articulate? In all seriousness, these are easy things to rationalise away. That’s pretty standard when I ask male patients about it. There’s nothing to glean from making guesses, though, and a semen analysis test will tell you something substantive as opposed to driving you to Google at 1 am.
Sign 3: That ache you keep telling yourself isn’t a big deal
The heaviness, swelling and/or nagging ache in the testicles comes and goes. Does it sound familiar to you? Many male patients endure these sorts of complaints far longer than they should before even mentioning them to a provider. Maybe it’s a varicocele, maybe it’s a bug. In any case, it’s certainly not a condition diagnosed by sheer procrastination.
Sign 4: An old injury that never really got a second thought
Tore something playing sports back in the day? Had a hernia fixed a few years ago? These things don’t always announce their consequences right away. Sometimes they sit quiet for years before showing up somewhere you weren’t expecting, like your fertility numbers.
Sign 5: Feeling off in ways that don’t seem connected
Tired constantly? Sex drive’s dropped off a cliff. Body hair’s changed in ways that seem random. None of this screams “fertility problem” on its own, but hormones run the whole operation behind the scenes, sperm production included. When one thing’s off, others usually follow.
Sign 6: Your daily habits, working against you without you realizing it
Hot tubs a few times a week. Tight underwear, all day, every day. Laptop parked directly on your lap for hours. Chemical exposure at work that you’ve just gotten used to. Heat and toxins don’t work fast, though. They work slow, and sperm cells don’t handle either one particularly well.
Sign 7: A childhood thing buried somewhere in old medical files
Undescended testicle as a baby, corrected early, never thought about since? Doctors still want to know about it. It’s one of those details that feels completely irrelevant now but can quietly affect fertility decades down the line.
None of this is meant to freak anyone out, and there’s genuinely nothing embarrassing about a semen analysis test. It’s just information. And information beats sitting there wondering, every single time. Go for early treatment, and you usually walk away with more options and a lot less stress.
